Register using your name, phone number, and email
Provide credit card details on our secure platform and confirm your booking
You will receive an appointment confirmation email/text and an INTAKE QUESTIONNAIRE link
Please complete the questionnaire and submit it at your earliest convenience
Kindly note that all patients are required to complete it prior to their appointment.
